The Centre for Complex Diabetes Care (CCDC) provides evidence‑based interdisciplinary medical, nutritional and psychosocial services to adults and seniors living with diabetes and other chronic health conditions. Intensive case management is provided until a patient is able to self‑manage their conditions with support from primary care or other services.
Reporting to the Manager, Centre for Complex Diabetes Care, the Psychologist works within an interdisciplinary team, collaborating with physicians, nurse practitioners, nurses, dietitians, social workers, physical therapists, and occupational therapists. Within the team, the psychologist offers conceptualization of patients' presenting concerns through a biopsychosocial model to enhance the quality of patient care and to improve clinical outcomes.
