Reconnect Health Services is a not-for-profit health service organization in the west end of Toronto. Our services include supports for seniors, caregivers, and people living with mental health and addictions concerns. Role

As a Case Manager, you will work with clients to achieve safe, realistic and reasonable goals within a complex health, social and fiscal environment. The Case Manager's primary function is the provision of quality health care and support services through the effective and efficient use of resources.
