Responsibilities

Case management involves coordinating all medical, paramedical, and psychological care to meet the needs and preferences of individuals living with chronic or complex conditions, including those with severe and persistent concurrent mental health and substance use disorders. The case manager is primarily responsible for:

• Multidisciplinary care coordination.
• Identifying clients who require an integrated care approach, particularly those with severe and persistent concurrent disorders.
• Patient advocacy.
• Liaison with external agencies.
• Sharing the care plan with the treatment team.
• Engaging family members and loved ones.
• Working across different sites based on organizational needs.
