FACILITY INFORMATION- Trauma Level: Level I Trauma Center
PRIVILEGES & COMPLIANCE- Credentialing Timeline: AZ Licensed: 6 weeks; IMLC: 2 months; Non-Licensed: 3 months (based on clean background). Credentialing can begin with pending license.
