This full-time, permanent role requires delivering holistic nursing care and education to adults with diabetes. Collaborating with a variety of health professionals, you’ll play a key role in addressing the barriers clients face in accessing health services while promoting overall well-being and chronic disease management.
Key Responsibilities:
• Provide comprehensive assessments and individualized client counseling
• Develop tailored health improvement strategies
• Maintain accurate documentation in electronic medical systems
• Conduct health education workshops for clients and groups
• Foster partnerships with community service providers
Requirements:
• BSc in Nursing with active registration as an RN in Ontario
• Preferred: Certified Diabetes Educator credentials
