The post holder will be responsible for providing clinical care for patients in community for the West Cluster in Salford Older Adult service. Additionally, the post holder will support and contribute to the development of the broader Older Adult team. The post holder will work with the Home-Based Treatment Team and Care Coordinators from the three Community Mental Health Teams in Salford to facilitate discharge and provide an alternative to inpatient admission.
The post holder will be a middle grade who is Section 12 approved and will provide two days of CT that is shared with the North cluster. The post holder will provide clinical supervision for the West patients; the North Consultant will continue in their role of clinical supervisor.
