Within the context of a client and family centred care model, the vision and values of the organization, the Home Health Dietitian works independently and in conjunction with the interdisciplinary team in client’s homes or in the ambulatory care setting to advocate, develop, implement, and evaluate nutrition care plans regarding oral and/or enteral nutrition support regime(s).
