About the Opportunity

The Clinical Dietitian is an integral member of the interdisciplinary health professional team and is responsible for the nutrition care, disease management and education of patients. The Dietitian initiates a cost effective nutrition therapy comprised of patient assessment, the development and implementation of a patient-centered, goal-focused care plan, with ongoing evaluation. Nutrition care plans are outcome-focused, incorporate evidence based best practice, are formulated to optimize both clinical and fiscal outcomes, and emphasize health promotion and disease prevention. The Clinical Dietitian is responsible for the continuum of care of patients with effective, evidence based strategies to self manage disease thereby decreasing the risk associated complications and hospital admissions.
