Discharge Planner – Halifax, NS

Role Overview

Reporting to the Manager, Inpatient Surgical Care/Child Life Services, the Discharge Planner provides leadership in planning and coordinating the timely discharge of patients from hospital. The role supports the Home First philosophy, performing home‑based care coordination assessments, resource allocation and authorization of continuing care programs to ensure smooth transitions from hospital to home across NSH zones. The Planner works closely with patients, families and multidisciplinary health‑care professionals, and coordinates with community‑based programs and services.
