INTENSIVE SERVICE COORDINATION CASE MANAGER

As a member of the Child and Youth Mental Health team, the Intensive Service Coordination Case Manager works in collaboration with other service providers to provide comprehensive intensive service coordination functions for identified children/youth with multiple/complex special needs in rural, remote, and urban communities. The successful candidate will develop, coordinate, and manage the implementation of an integrated service plan to meet the identified needs of the child/youth and family.

JOB REQUIREMENTS

• Graduate or Baccalaureate degree in Human Services. A College Diploma in Social Services may be considered along with other relevant university degrees or diplomas deemed suitable by the Executive Director.
• Experience working with children, youth and families is considered an asset.
