Job Overview

As a member of the Child and Youth Mental Health team, the Intensive Service Coordination Case Manager collaborates with other service providers to provide comprehensive intensive service coordination functions for identified children and youth with multiple or complex special needs. This includes developing, coordinating, and managing the implementation of an integrated service plan to meet the identified needs of the child or youth and their family.
