Job Description Key Responsibilities: - Provide direct nursing care to residents including medication administration, wound care, and monitoring of vital signs.
- Assist in the development, implementation, and evaluation of resident care plans.
- Observe and report changes in resident conditions to the RN or physician.
- Document nursing care, resident status, and responses to treatment accurately in the medical record.
- Ensure safe and effective communication between residents, families, and the interdisciplinary care team.
- Maintain a clean and safe environment for residents and staff.
- Supervise Certified Nursing Assistants (CNAs) and delegate tasks appropriately.
- Participate in shift hand-offs, care conferences, and staff meetings.
- Comply with all federal, state, and local regulations, as well as facility policies and procedures.
