Partners Community Health (PCH) is a new not‑for‑profit organization focused on delivering healthcare services in Mississauga and West Toronto. PCH operates two new state‑of‑the‑art LTC homes and a community hub in West Mississauga with a combined capacity of 632 beds. Key Responsibilities

• Formulate differential diagnoses through the integration of resident information, evidence‑informed practice, and determinants of health in the assessment, diagnosis, and therapeutic management of residents within a team of physicians, nurses, PSWs and several other interdisciplinary team members.
