Parkdale Queen West Community Health Centre (Parkdale Queen West CHC) is a dynamic, community-based health care organization serving mid‑west downtown Toronto. Offering a broad range of services, including primary health care, dental care, harm reduction, health promotion, counselling, and community development programming, we are also committed to serving people who face barriers to health care and to reducing those barriers. PQWCHC is a multi‑site organization.

General Responsibilities

Reporting to the Manager of Clinical and Allied Health, Parkdale site, this position is responsible for providing holistic health care to clients accessing care through PQWCHC’s Mobile Health Services. This may include providing care to individuals in buildings where they reside and in partnership locations. The NP also acts as a supervisor to health care students and participates in research and quality improvement activities.
