Job Description
Provide high‑quality nursing care to patients in the community with a focus on wound, ostomy, and continence management.
- Conduct comprehensive assessments and develop individualized care plans for patients with wound, ostomy, and continence concerns.
- Provide patient‑focused, safe, holistic nursing care and work collaboratively with other members of the team.
- Ensure nursing care is based on current and best practices to achieve successful outcomes.
- Continually assess, anticipate, identify and respond to new and changing patient needs.
- Travel between patients’ homes within a specific geographical region.
Qualifications
- Must have completed NSWOC or International Interprofessional Wound Care Course (IIWCC).
- Currently registered and in good standing with the College of Nurses of Ontario (CNO).
