Take on an impactful role as an Occupational Therapist in the GAIN Community Team. Focus on improving the quality of life for older adults as a part-time, on-site practitioner.
As an essential part of the GAIN Community, you will participate in Comprehensive Geriatric Assessments while performing clinical triage for new referrals. Your expertise will be essential in gathering data to develop individualized care plans aimed at enhancing daily living activities and maintaining independence among frail seniors.
Key Responsibilities:
• Engage in inter-professional Comprehensive Geriatric Assessments
• Conduct clinical triage for incoming referrals
• Gather comprehensive data in all Domains of Assessment
• Offer clinical consultation for daily living improvements
• Analyze senior capabilities through interviews and assessments
Requirements:
• Degree in Occupational Therapy mandatory
