Extendicare is seeking a part time Registered Practical Nurse (RPN) to join their interdisciplinary team. In this role, you will focus on person-centered care and effective communication with residents and families.

The RPN will report to the Director of Care and is responsible for assessing residents’ clinical needs and implementing care plans. You'll work collaboratively with healthcare professionals and ensure compliance with regulatory standards. This role emphasizes strong communication, critical thinking, and care coordination skills.

Key Responsibilities:
• Complete clinical care assessments and service plans
• Implement and evaluate care effectiveness and adjustments
• Provide direct care and medication administration
• Manage residents’ care assignments for efficiency
• Respond to medical emergencies and significant condition changes
