Responsibilities

• Perform assessments, diagnosis, treatments, health education/counselling, and other medical functions for primary care clients.
• Conduct same‑day access appointments, home visits as required and participate in the Centre’s on‑call services on rotation to provide after‑hours services.
• Use an equity and client‑centered approach, engaging clients and collaborating with other services mindful of social determinants of health.
• Participate in continuous quality improvement activities.
• Participate in case conferences, make referrals to interdisciplinary teams and external resources, and correspond with a collaborative team.
