As a key member of Fraser Health, you will engage in trauma-informed practices while performing comprehensive psycho-social assessments and developing individualized care plans for patients. In addition, you'll collaborate with medical staff to improve patient flow, advocate for community resources, and ensure timely admissions and transfers.
Key Responsibilities:
• Conduct in-depth psycho-social assessments for patients
• Develop and modify care plans with interprofessional teams
• Provide crisis intervention and counselling to individuals
• Advocate for patient rights, dignity, and support
• Facilitate discharge planning and aftercare referrals
Requirements:
• Graduation from an accredited School of Nursing
• Current registration with BCCNM as RN or RPN
