Right Siting Officer, PHIC - Strategic Planning and Evaluation
The Right Siting Officer is responsible for assisting with navigating patients to the appropriate downstream care services available in the community. They will work closely with physicians, nurses, administrators and actively liaise with community-based care providers to facilitate the transfer of patients from one care site to the next, based on patients’ care needs and social and financial circumstances.
Responsibilities
- Provide assistance to clinicians by explaining the available options for follow-up in the community. Address clinicians’ queries about patients’ discharges and follow-ups.
- Assess patients’ needs and care requirements and recommend appropriate community-based services as appropriate.
