The Community Connector connects older adults 60+ in Richmond to appropriate community-based programs, services, and supports to improve their health, wellbeing, and independence.
Key Responsibilities
- Maintains an active caseload of seniors with short-term needs through referrals from health care professionals and community agencies
- Identifies individuals needing support to link them to organizations providing services to older adults
- Completes client intake process to assess abilities, needs, and risks
- Develops routines, structures, and resource referrals to reduce risk of frailty and social isolation
- Builds and maintains networks within the community and strengthens relationships within the community-based seniors serving sector
