Job Title: Social Worker – Adult Safeguarding

Rate: £35 per hour

What will your responsibilities be?
In this essential role within Adult Safeguarding, you will be responsible for carrying out social work assessments and providing advice and guidance to service users. You will liaise and work closely with contacts from different sectors, including health and the voluntary sector, as part of a multi-disciplinary team. Ensuring effective communication and sharing knowledge and best practices will be key. You will review and monitor care plans and the quality of service provision, responding to changing needs. This includes developing and evaluating reablement programs as part of care packages to support independence.
