Supervisor, Home and Community Care

This is a full‑time, permanent supervisor position with the Department of Health in Nunavut, reporting to the Manager Home and Community Care. The role involves delivering nursing care and support services across communities, enabling residents to maintain independence.

Responsibilities

• Assess the status of clients admitted to the Home and Community Care Program.
• Develop, modify, and implement individual client care plans based on assessed needs and prescribed medical regimes.
• Supervise, train, and manage Home and Community Care staff assigned to various communities.
• Collaborate with the Supervisor of Health Programs and Community Health Nurses to coordinate care.
• Ensure services promote optimal functioning in activities of daily living and enable clients to remain in their homes.
